The purpose of the "ears" as you describe them is to transfer the lift on the deck caused by halyard turning blocks mounted to the top of the mast collar or in its vicinity. These blocks can pull up on the deck w/ considerable force. The "ears" clamp the deck collar to the mast so the net force on the whole structure is zero.
Now, why are they misaligned?
Sometimes, when the mast is pulled for service, the heal is so corroded, that some of it has to be cut off. Unless the mast step is built up an equivalent amount, the mast would drop in relation to the deck after such a repair and result in this misalignment.
An alternate explanation is the deck has been "lifted" by the continual pull of the turning blocks. (I think less likely)
